The Policy Odyssey

Charting the course of a digital health company has always been an Odyssey: markets shift, technologies evolve, and customer needs change. Policy is also a part of that voyage, with its own fair winds and “siren songs” alike. Last week, the White House released a new report, Science: A New Golden Age, outlining a vision towards an “AI-driven scientific revolution” with policy levers in place to bolster scientific and healthcare capabilities in the United States. To pull it off, roughly $200B in annual federal R&D funding would be reoriented toward individual scientists, new funding mechanisms, and AI-focused labs—de-emphasizing the role of legacy institutions (e.g., universities, funding bodies) that have long anchored U.S. scientific progress. Through initiatives such as the Genesis Mission and Operation TrialBlazer, the federal administration is focusing on domestic commercialization efforts via government-industry partnerships that accelerate AI drug discovery and modernize clinical trials.

That broader vision is extending across healthcare agencies as a part of other initiatives, too. Last week's FDA advisory committee vote on wellness peptides further signaled support for domestic innovation in lieu of reliance on overseas markets. The FDA also opened a grant program for digital health technologies in drug development and selected Dexcom as the first participant in its TEMPO pilot, pairing continuous glucose data with CMS's ACCESS payment model to generate real-world evidence for Medicare. But not every wind is at founders’ backs. CMS' recently proposed changes to remote patient monitoring would require clinical staff to be employed by the billing practice rather than third-party vendors. If finalized, many existing partnerships between providers and remote patient monitoring companies would no longer qualify for Medicare reimbursement, pressuring health systems to bring those capabilities in-house or discontinue them.

For digital health companies, policymakers are creating new routes for innovation while simultaneously narrowing others. The destination is ambitious: innovations that “catalyze the greatest scientific returns.”
